This App facilitates the Google Cloud Storage and PubNub Data Network consumer-directed services that provides users with the capability to communicate securely. Your use of this site is a record of your permission and consent to collect, store, and manage records including any personal health information on the Google Cloud and PubNub data network. This may include, but is not limited to, personally identifiable information, such as name, address, phone number, and e-mail address, and Protected Health Information (as defined under the Health Insurance Portability and Accountability Act).

We use the information You provide to facilitate Health System and on occasion to:
• Administer the Site and diagnose problems with the server;
• Verify Your identity; and
• Provide authorities or relevant individuals with information as required by law.
